Care Service Specialist (CSS) 
Bellefonte, PA

 Under the direction of the Site Manager, the Care Service Specialist is responsible for assessing consumer care, the safety needs with their Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to maintain the consumer in their home as outline in the Service Authorization Forms (SAFs).

Job Duties/Responsibilities:
(May include but are not limited to)

  • Maintain accurate and detailed reports and records such as care plans, visit notes and client intake paperwork in accordance with ProStat's policies and procedures
  • Monitor, record and report any and all changes in the consumer's physical, emotional and/or mental condition
  • Adhere to the Health Insurance Portability and Accountability Act (HIPPA) privacy policies and procedures
  • Serve as a liaison between DCWs and consumers
  • Maintain confidentiality of all information pertaining to the consumer, the consumer's family, the Direct Care Workers (DCWs), co-workers, community providers and referral sources
  • Schedule and conduct 90-day visits with both consumers and DCWs
  • Assist with DCW training and evaluations
  • Assuming the role of a DCW for consumers in emergency situations and perform on-call duties outside normal office hours as needed
  • Communicating issues with the Site Managers and Support Coordinators
  • Building and encouraging mutual trust, respect and cooperation among co-workers, consumers and DCWs

Skills and Qualifications:
(May include but are not limited to)

  • Basic office and computer skills
  • Basic medical terminology knowledge 
  • Excellent oral/written communication, active listening and comprehension skills
  • Strong organizational, coordinating and time management skills
  • Ability to work independently, demonstrate sound judgement, decision making and problem solving skills
  • Ability to multitask and work effectively under pressure in a fast-paced environment
  • Ability to lift, pull and push a minimum of (50)lbs

Education and Experience:

  • High school diploma or GED
  • Minimum of (1) year office/administrative experience 
  • Minimum of (1) year relevant experience with a related organization and/or private duty care 
  • Valid Pennsylvania Certified Nursing Assistant (CNA) certification or License Practical Nurse (LPN) license is preferred but not required
  • Bilingual preferred but not required
